I/WE, the parents of the above named child, hereby give my/our approval to take part in all activities of West Niagara Minor Baseball. I/We are aware of the inherent risks involved in participating in such activities. They include on the field activities and also include the transportation to and from the activities. I/WE do hereby waive, release, resolve, indemnify and agree to not hold West Niagara Minor Baseball, the organizers, sponsors, supervisors, participants and persons transporting my child to and from these activities, for any claim arising out of an injury to my/our child, whether the result of negligence of for any other cause, except to the extent and in the amount covered by accident or liability insurance.
